Fill out our short Getting Started form for behavioral health (BH). Once you’ve completed the form, be sure to submit it by clicking the button at the bottom.
Once your form has been processed, you will receive a confirmation email that your child has been added to our BH waitlist.
OCCUPATIONAL / PHYSICAL THERAPY
Get Started with OCCUPATIONAL/PHYSICAL THERAPY
Occupational Therapy (OT) and Physical Therapy (PT) services require a referral from your child’s doctor. Please contact your pediatrician to request a referral.
Once we receive your referral, a member of our team will reach out to you to confirm you have been added to the waitlist.
